Amy Williams, M.A. CCC-A Dear Amy Williams, RE: Lacey Fields This letter introduces Lacey Fields. I have been her pediatrician since her birth four months ago. Lacey lives with her parents, John and Andrea Fields, in Huntington. Mr. Fields owns a bread route, and Andrea is a homemaker. Mr. Fields became concerned earlier this month that Lacey was not responding as he thought she should to noises in her environment (Gordon-Langbein, n.d.). Mr. and Mrs. Fields brought Lacey in for an examination a few days after Mr. Fields voiced his concerns. I performed a basic examination of the child and determined, based on my findings and the parental concerns, that Lacey should have a more thorough hearing evaluation (Gordon-Langbein, n.d.). Lacey Fields is being referred to you for a formal hearing evaluation. She was a born at 38 weeks gestation in good health. She weighed five pounds six ounces at birth. She had a one minute Apgar score of six and a five minute Apgar score of eight. Mrs. Fields did not experience any difficulties during pregnancy or delivery. Lacey has been seen in my office four times since birth. She came in at two weeks, one month, two months, and four months for check-ups. No abnormalities were noted during these visits. She has been in good health and is growing according to established norms. After your consultation with the Fields', please provide me with information regarding any impairment of Lacey's hearing abilities. If impairment is noted, I have advised Mr. and Mrs. Fields to immediately make an appointment with an SLP, either in your practice or another office (Schow & Nerbonne, 2002).
